Facilities Use Request Form Process
1. Read External Use Lease Agreement Terms and Fees.
2. Complete the Online External Building Use Form (Below).
3. Obtain proper liability insurance for building use/print policy for documentation.
4. Form must be submitted no less than 14 days prior to your event.
5. You will be contacted after submitting the form to confirm availability of the building.

Facilities Use Lease Agreement Terms
I. Parties: This Lease is entered into by and between the U.S. Grant Career Center School District Board of Education, hereinafter referred to as "Lessor", and the name entered below hereinafter referred to "Lessee".
II. DESCRIPTION OF FACILITIES AS LEASED - Lessor does hereby lease to Lessee the following described portion of the U.S. Grant Career Center School:
III. Lessee will provide police protection at the lessee's expense.
IV. Lessee agrees that it will not use the leased premises for any purpose other than that which is contained in this Agreement without it first obtaining the written consent of Lessor.
V. Lessee agrees that it will observe and comply with the policies and regulations of the Lessor while using the leased premises and shall be responsible for the conduct of all persons on the leased premises as a result of Lessee's use of the facilities. In addition, Lessee agrees that it will not permit the use of tobacco, alcohol or drugs, nor will it permit any games of chance or gambling, at or on the leased premises.
VI. Lessee agrees that it will be responsible for providing adequate supervision when utilizing the leased premises.
VII. Lessee agrees that it will be responsible for any damage to the leased premises or its contents as a result of Lessee's use.
VII. Lessee agrees that it will maintain insurance covering its use of the leased premises in an amount equal to $1,000,000. Lessee agrees that it will provide Lessor with proof of insurance at the time of the execution of this Lease Agreement. Lessee agrees that it shall indemnify, defend, and save Lessor, its agents, and employees harmless of and from every kind of claim, suit, or action, arising or growing out of Lessee's use of the premises.
IX. Lessee shall not assign or transfer its interest in this Lease to another without the prior written consent of Lessor.
X. Lessee agrees that if the Board of Education determines it needs the leased premises for school purposes, Lessor may cancel this lease by giving Lessee five (5) days written notice of that fact.
XI. If Lessee shall fail to pay the rent when due or shall fail to comply with any of the other terms and conditions of this Lease, then at the option of the Lessor, this Lease may be terminated and cancelled immediately. This Lease shall, in all respects, be governed by the laws of the State of Ohio.
XII. This Lease expresses the entire understanding of the parties. No modification, amendment, or alteration hereto shall be valid, except as stated in writing and signed by both parties.
XIII. If school is cancelled on the date of a scheduled event because of calamity, natural disaster, lack of electrical power, or for any other reason, the event shall also be cancelled and any rent paid to Lessor shall be returned to Lessee. Lessee shall remain responsible for the payment of any perishable product (i.e. flowers, food, etc.) ordered from Lessor that has been prepared by Lessor for the event.
